OpenNotes: Providing Patients Access to Physicians' Notes

This presentation was made possible through the  Partnership for Patients initiative,  a nationwide public-private collaboration that began in 2011 to reduce preventable hospital-acquired conditions and 30-day readmissions. It features results of an OpenNotes study conducted prior to the presentation, measures of success and more patient & family engagement resources from IHA.
